Medical Exam Forms
If you are a new California State University, Northridge student-athlete (freshman or transfer student) or are a returning student-athlete, you are required to fill out one of the Medical Packets that are linked below. You are also encouraged to read the informational handouts below.
Please return your completed and signed medical packet to the Sports Medicine Department before August 1.
