INCOMING FRESHMEN / TRANSFER STUDENT-ATHLETES PRE-PARTICIPATION PHYSICAL EXAMS
• All California State University, Northridge student-athletes are required to complete a pre-participation physical examination prior to participating in any strength and conditioning session, practice, game, and/or competition.
• All pre-participation physical examinations must be completed by a California State University, Northridge Team Physician.
• Student-athletes must complete the Incoming Student Athlete forms and demographics on Sportsware prior to their physical examination.
• All student-athletes must submit a photocopy (front & back) of their health insurance card.
• Your coach will notify you as to the date and time of your physical examination.
• All information must be complete no later than August 1st
